COMPATIBILITY CALL

This is a 30-minute Compatibility call with our community Liaison. We analyze your current situation and systems to determine if your agency qualifies for our premier program. If qualification metrics are met, onboarding will be scheduled. Ensuring your agency understands the levels of care our caregivers can provide is key to reduce wait time for homecare services

REFERRAL/TRUSTED PARTNER ONBOARDING

We're here to help you get up and running. We'll take you through a quick onboarding process, so that we can seamlessly plug into your processes and become an extension of your team allowing for a quick and confident decision— either acceptance or denial (Providing clear reasons for denying a patient)

NEEDS ANALYSIS AND CUSTOM CARE PLAN PRE-ADMISSION SCREENING

Once we have the documents and information we need. We make in-home needs assessments to help clients determine their own requirements and assist them in developing suitable care plans.

By matching the patient with the right caregiver, we help reduce the likelihood of a readmission

The importance of a thorough pre-admission screening cannot be overstated.. Of course, doing this will add time to the process and could cost us a referral, but not doing it could end up costing so much more
